Wondering how to get reimbursed for items you bought on behalf of a PTA program? We’re trying to make it easier. Please see below for instructions.
Don't forget, PTA is a 501(c)3 non-profit and can NOT reimburse you for sales tax! You can find a copy of our tax exempt form here. Please use it for all purchases! If needed, our Tax ID number is 74-6085722.
Remember to upload a copy of your receipt/invoice to the google form. Or email a copy to treasurer @summitteagles.org. Please indicate what budget your check should come out of, ie. Teacher wish list, appreciation, grade level parties, Eagle Fest, etc.
If you’re a teacher or staff member, your check will be delivered to your mailbox at the school. All other checks will be delivered to the address provided. If you’d like to make other arrangements for pickup, please include that in the comments section of the form.
